Are you currently accepting new patients?

We are currently accepting new patients and often have same day appointments available. Please call our front desk at 208-344-6080 to inquire.

What do I need to bring with me for my first appointment?

Please arrive 10 - 15 minutes early to your first appointment so that you can complete all of the required forms. Please bring your insurance card with you.

What can I expect during my first appointment?

You can expect to be asked a number of medical and non-medical related questions. We like to get to know you during your first appointment. We will discuss personal and family medical history, current medications, social history, such as "What do you do for a living? What do you like to do for fun? What is your diet and exercise routine like?" etc. You can also expect a full head to toe physical. We also may recommend ordering routine blood work, depending on your age, gender, medical history, etc. 

What insurances do you accept?

We currently accept most major forms of private insurance, such as Blue Cross, Blue Shield, Select Health, United Healthcare, Aetna, Pacific Source, etc. We also accept Tricare, Medicaid and Medicare. 

Is there a sliding fee scale for out of pocket payers?

Yes! For those without medical insurance we offer a sliding fee scale. Please call our office to discuss your current situation and needs. 

How do I sign up for/use the Patient Portal?

Talk to the front desk about how to sign up for the patient portal. Once signed up, you can simply click on the "Patient Portal" link on our home page or on the "Portal" tab in the upper right hand corner of our website.

What are your clinic hours?

We are open Monday - Friday, 8:00 am to 4:00 pm.

How do I schedule an appointment?

Currently we are unable to schedule online. If you are a current patient you can request an appointment in the portal.

How do I check my records?

You are able to sign into the portal and access your records, make requests and send records electronically. If you do not have portal access please call our front desk and we will get you set up.

How can I pay my bill?

Use the “Pay my bill” button on our homepage to take you to the payment page, or the “Pay my bill” in the website menu. This will take you to a payment page.

How do I request a prescription?

Call your pharmacy first for prescription that have been filled in the past. They will send an electronic request to the the office. If for some reason this is not possible please call our office so we can troubleshoot.

Where do I find forms for school?

Please find all forms here. Print out completed forms and bring to your appointment.

How do I send a message to the front desk?
